Uberic Form Notifications is a Google Workspace add-on that allows you to define custom workflows for your Google forms.

You can use the add-on if you have an organizational account or a personal account with Google.

With Uberic Form Notifications, you can:

• send custom notifications to any email addresses that you specify;
• send on behalf of another Gmail account that has been added to your primary Gmail account;
• specify the Reply-To email address in the notification message;
• define a custom notification template;
• attach a PDF file that contains the form response data;
• attach files on Google Drive to the notification;
• define arbitray notification rules based on user input;
• notify the form respondent.
